Family Supports Waiver (FSW)

At Pure Care Residential LLC, we provide personalized, non‑residential services and supports to children and adults with developmental disabilities, helping them live successfully and safely in their homes and communities.

FSW Services We Provide

Tailored supports based on your person‑centered plan.

Transportation

Safe, reliable rides for approved medical appointments, work, day programs, and community activities.

Life Skills Coaching

Coaching and practice for everyday skills: cooking, cleaning, money management, personal safety, and community navigation.

Participant Assistance & Care (PAC)

Direct support with activities of daily living, health‑related tasks under RN direction, and household routines.

Community‑Based Habilitation

Person‑centered instruction (individual or group) to build social, communication, and independent‑living skills in community settings.

Behavior Support

Assessment, plan development, and ongoing coaching using positive behavior interventions that promote safety and success.

Who Qualifies for FSW?

The Family Supports Waiver is an Indiana Medicaid waiver administered by the Bureau of Developmental Disabilities Services (BDDS). Eligibility generally requires a documented intellectual or developmental disability and meeting Medicaid financial and level‑of‑care criteria. Enrollment may be subject to BDDS waitlist and state availability.
